There is also a consent form, BOTH need to be signed.
Nebraska Statute requires a physical examination by a NEBRASKA physician if you are:

An incoming Freshman (gr.9).

An incoming 7th grader with an updated immunization report.

Transfer students.
-
An incoming kindergartener with an updated immunization report.

Participating in any Sport, PE, and/or Weights class.

School Facility Use Request
ATTENTION!
Attention families of graduates from 2021. School districts are required to retain Special Education records for a period of five years following the completion of a student’s educational program. Students who have graduated/completed their program in 2021 will now have their Special Education records destroyed as they are no longer valid. If you would like to obtain these records, then please contact Staci Holthus at staci.holthus@thedfordschools.org or 308-645-2614. They will be forwarded to you. If no contact is made by Friday, May 7, 2027, then such records will be destroyed.
